***If you do not see your insurance company listed here, please contact your insurance carrier and inquire as to who administers their mental/behavioral health coverage. It is often the case that the above listed companies are contracted with insurers like Blue Cross/Blue Shield and Cigna, et al. to provide their members with mental and behavioral health coverage.

We will be happy to prepare an insurance claim for you with an envelope and stamp. All you have to do is drop it in the mail!
Please be aware that your insurance company may not offer reimbursement for medical treatment rendered by an out-of-network physician. We encourage you to call your insurance company to determine what out-of-network benefits you may be entitled to before visiting our clinic.
